To make your reservation without on-line booking, please telephone us (toll-free from the U.S.) or E-mail us to inquire about availability of the desired accommodation on the dates you prefer. Then print this page, fill in the blanks, (please remember to sign the form!), and FAX it to us. We will then promptly confirm your reservation! MAUI TRADEWINDS RESERVATION REQUEST & CREDIT CARD AUTHORIZATION

If my reservation is for less than fifteen nights, I understand that there is no payment required in advance of my arrival date, but a credit card authorization is necessary in order to hold my reservation. The credit card authorization is used to guarantee payment, but my card will not be charged except in the event of cancellation or accidental property damage, as set forth in the Reservation and Financial Understanding which I have read. Maui Tradewinds may run an "authorization only" to verify my credit -- this is not a charge to my account and will not appear on my statement. Payment in full will be made by Traveler's Checks, Certified check or cash at the time of arrival, unless other arrangements are agreed-upon in advance. 3. In the event of cancellation, if Maui Tradewinds receives my written notice at least sixty days ahead of my scheduled arrival date, Maui Tradewinds is authorized to charge my credit card in the amount of $__________, which is 50% of the total rental amount.
